Caremore Care Coordinator - Washington D.C

The Care Coordinator, CareMore Primary Care is responsible for assisting case managers to coordinate the clinical care for the Medicaid and/or Medicare population across the continuum of care. The Care Coordinator will be engaged directly with members and will support Care Managers in, developing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum and ensuring member access to servicesappropriate to their health needs. He/She will report to the Case Manager.

Essential Duties and Responsibilities

● Implements, coordinates, monitors, and evaluates options and services to meet patient’s health needs andensures appropriate use of clinical resources ● Maintains daily admissions and discharge records for hospitalized patients (medical and behavioral health) ● Expedites and coordinates appointments for assigned hospitalized patients ● Receives incoming patient calls for the case management department and appropriately facilitates calls ● Collects relevant clinical information and submits authorizations with payer partners ● Coordinates care and follow-ups with patients to ensure they receive needed services ● Keeps providers involved with patient’s care updated on appointments, condition, and additional clinical support needed by leveraging EMR communication, interdisciplinary team meetings and face-to-face communication ● Maintains accurate and complete documentation in the EMR ● Responsible, under the direction of the Care Manager, to view and document higher level of Medicaidqualifications, submit appropriate qualifications to state and track completeness

  • Requires a high school diploma

  • Experience in Care Management in a care delivery setting; or any combination of education and experience,which would provide an equivalent background

  • Strong knowledge of medical vocabulary required

  • This position requires strong communication skills

  • Proficient with computers and common programs

  • Can work in the office everyday

  • Medical Assistant training or knowledge is a preferred
